Home health care includes a wide range of services you can get in your home for an illness or injury. Home health care is usually less expensive, more convenient, and just as effective as care you get in a hospital or Skilled Nursing Facility (SNF).

Home Health Aide
Part-time or intermittent home health aide care (only if you’re also getting skilled nrusing care, physical therapy, speech-language pathology services, or occupational therapy at the same time), like:
- Help with walking
- Bathing or grooming
- Changing bed linens
- Feeding
Medicare doesn’t pay for:
- 24-hour-a-day care at your home
- Home meal delivery
- Homemaker services (like shopping and cleaning) unrelated to your care plan
- Custodial or personal care that helps you with daily living activities (like bathing, dressing, or using the bathroom), when this is the only care you need
Long Term Care Program Covered by Medicaid
At Ameri-Care Professional Service, Inc, we proudly serve as a participating provider of Home and Community-Based Services HCBS, which are Medicaid-funded long-term care programs that deliver medical, personal, and supportive services directly in a person’s home or community to help them avoid the need for institutionalization. These flexible, state-managed programs assist with essential daily activities such as bathing and dressing, as well as healthcare services like nursing and therapy, all tailored for seniors, individuals with disabilities, or those with chronic conditions with a strong focus on promoting independence and enhancing community integration.
Key responsabilities:
PERSONAL CARE
- Assist with bathing, grooming, hair/skin care, and oral hygiene.
- Help with dressing, undressing, and changing bed linens.
- Provide toileting and incontinence care.
- Assist with mobility, transfers (bed to chair), and simple prescribed exercises.
- Offer medication reminders (not administration, unless state-permitted).
HOMEMAKING/HOUSEHOLD TASKS
- Plan, prepare, and serve meals according to dietary needs.
- Do laundry, dust, vacuum, and light cleaning.
- Shop for groceries and household supplies.
COMPANIONSHIP & ERRANDS
- Provide social engagement and emotional support.
- Arrange and provide transportation to appointments, shopping, and outings.
REPORTING & SAFETY
- Monitor and report changes in the client’s condition, behavior, or environment.
- Maintain a safe, clean, and secure home environment.
ADMINISTRATIVE SUPPORT
- Help organize schedules and plan appointments.
- Communicate with the agency/supervisors about client needs.
